The 2026 ranked list is mostly resolved now. Ultragenyx’s UX111 got approved as Fayuvi. Ionis’s zilganersen got approved as ZANVASTRO. Sarepta’s contrarian thesis re-rated, then gave some of it back. Replimune’s RP1 finally cleared the FDA. Four of seven picks played out, and the survivors now face a different question: what is their second shot in 2027?

That is the frame for this list. 2027 rewards companies whose 2026 catalyst already resolved and who have another binary coming, not the one-and-done trades that double or halve on a single decision. The full 2027 FDA calendar has the PDUFA dates. This list ranks the platform companies with a second shot on goal. Every name carries a specific 2027 catalyst, a public comp, and a number behind the thesis.

1. Ionis (IONS) | $44.50 | Market Cap $7.4B | Ulefnersen FUS-ALS filing

Ionis is the cleanest second-shot story on this list. It won its first approval of 2026 on September 3 when zilganersen became ZANVASTRO for Alexander disease. Then on September 22 it dropped the Phase 3 FUSION topline for ulefnersen in FUS-ALS: the primary endpoint met with p=0.0005 on a joint-rank analysis of function and survival. That is the first placebo-controlled trial ever to show a benefit targeting the genetic cause of FUS-ALS, a subtype that is only 0.6 percent of ALS cases. Otsuka runs the regulatory push, and a 2027 filing is the near certainty. The FUSION data is the anchor.

The comp is Alnylam at roughly $34 billion, trading near 15x revenue. Ionis at $7.4 billion generates over $1 billion in annual revenue across QALSODY, SPINRAZA royalties, WAINUA, and olezarsen. Each antisense approval compresses that gap. Ulefnersen is a niche drug, not a $1 billion-a-year drug, but it is the third validation of the platform in a year. That is what re-rates the stock.

2. Sarepta (SRPT) | $18.67 | Market Cap $1.97B | AMONDYS 45 and VYONDYS 53 sNDAs plus siRNA data

Sarepta is the contrarian that has not fully played out. The stock re-rated from $15.43 to the low $20s, then faded back to $18.67. At $1.97 billion the market is pricing a franchise that generated $731 million in a single quarter at roughly 0.7x annualized revenue. The ELEVIDYS DMD franchise is real but scarred by a black box warning and a 36 percent workforce cut.

The 2027 catalysts are concrete. The FDA accepted sNDAs for AMONDYS 45 (casimersen) and VYONDYS 53 (golodirsen) on June 30, 2026, putting early 2027 decisions on the PPMO exon-skipping franchise. The first clinical data from the siRNA pipeline, targeting FSHD1 and DM1, arrived March 25. The comp that matters is Dyne at roughly $4 billion pre-revenue versus Sarepta at under $2 billion with a commercial franchise. That gap is the thesis. If the sNDAs convert and the siRNA data holds up, Sarepta re-rates; if Dyne’s rostudirsen takes share, the franchise erodes further. See the Dyne BLA analysis for the other side.

3. Viking (VKTX) | $35.56 | Market Cap $4.15B | VANQUISH Phase 3 readouts

Viking is the scarcest asset in biotech: an independent, late-stage obesity company. VK2735 is a dual GLP-1/GIP agonist in two formulations. Subcutaneous VK2735 hit up to 14.7 percent weight loss at 13 weeks with no plateau. Oral VK2735 hit up to 12.2 percent in the VENTURE-2 study. Both are now in Phase 3: VANQUISH-1 enrolled 4,650 patients with obesity, VANQUISH-2 enrolled roughly 1,000 with type 2 diabetes and obesity, and the oral program is slated to start Phase 3 in Q4 2026. Topline VANQUISH data lands in 2027.

The comp is the obesity market itself, sized in the GLP-1 market piece. Novo Nordisk and Lilly are worth hundreds of billions on the back of this class. Viking is the one name where an acquirer could bolt on a late-stage oral-plus-injectable obesity franchise at a digestible price. The risk is the data: tirzepatide set the bar near 20 percent weight loss, and if VANQUISH prints below it, Viking gets cut. But at $4.15 billion for the second-most-advanced independent obesity program, the optionality is underpriced.

4. Telix (TLX) | $11.45 | Market Cap $3.89B | Three registrational readouts

Telix is the radiopharma pure play. The imaging business funds the therapeutic pipeline: Illuccix drove $247 million in Q2 revenue, up 21 percent year over year, against full-year guidance of $950 to $970 million. The Telix-ITM merger added three late-stage therapeutic assets: TLX591-Tx in prostate cancer, TLX250-Tx in kidney cancer, and TLX101-Tx in glioblastoma. Those registrational readouts are the 2027 catalysts.

The comp is Novartis, which runs a $3.5 billion annualized radiopharma franchise on a $300 billion market cap and does not move on a single drug. Telix at $3.89 billion, growing revenue 20 percent plus, is the pure play on the same theranostics arc. The risk is that imaging agents face the same generic and academic-center pressure that compressed Lantheus’s PYLARIFY, and the therapeutic pipeline is earlier stage. But a near-$1 billion revenue business at roughly 4x forward sales, with three shots at therapeutic data, is the Buy in this sector.

5. Alnylam (ALNY) | $255.96 | Market Cap $34.2B | Zilebesiran and nucresiran Phase 3

Alnylam is the quality anchor. AMVUTTRA (vutrisiran) did $1.01 billion in Q2 2026 revenue, up 106 percent, and the RNAi platform is the largest in the sector. The 2027 catalysts are zilebesiran, the hypertension RNAi in Phase 3, and nucresiran, the next-generation ATTR-CM program. A successful zilebesiran readout would open a cardiovascular indication far larger than anything Alnylam has addressed.

The honest caveat: this is a $34 billion company trading near 8x revenue. It will not double on any single readout. The comp is argenx at 13x sales, which shows what the market pays for a franchise still growing triple digits. Alnylam is the safe multi-shot platform, not the asymmetric re-rate. Own it for the pipeline; size it for the valuation.

6. REGENXBIO (RGNX) | $7.40 | Market Cap $488M | Sura-vec wet AMD Phase 3 data

REGENXBIO is the highest-risk, highest-reward gene therapy name. Sura-vec (ABBV-RGX-314) is an AAV8 gene therapy for wet AMD partnered with AbbVie. The ATMOSPHERE and ASCENT registrational trials enrolled over 1,200 patients, the largest gene therapy program ever run, and topline data lands in Q4 2026. Five-year follow-up showed stable to improved vision with reduced injection burden. The sura-vec primer has the trial detail.

The 2027 catalyst is the BLA, assuming the Q4 readout hits. The comp is Regeneron’s Eylea at $5.9 billion in 2024 wet AMD revenue; even 10 percent of that is $590 million, more than RGNX’s entire market cap. The risk is structural: gene therapy for a common eye disease is unproven, AbbVie takes the economics through milestones and royalties, and a Q4 miss is a 60 to 70 percent drawdown. This is a binary sized for risk capital, not a core holding.

7. uniQure (QURE) | $38.23 | Market Cap $2.65B | AMT-130 Huntington’s BLA

uniQure is a second-shot story in the making. AMT-130 is an AAV5 gene therapy for Huntington’s disease, a fatal condition with no disease-modifying therapy. The Phase 1/2 data in 29 patients showed statistically significant slowing on the composite UHDRS versus an external control, and the FDA agreed in June 2026 to accept it as the basis for an accelerated approval BLA. The submission went in during Q3, with a decision likely in mid-2027. The AMT-130 BLA coverage tracks it.

The risk is the dataset: 29 patients versus an external control is not a randomized Phase 3, and the FDA is unpredictable on single-arm gene therapy filings. But a Huntington’s disease-modifying approval would be historic, and at $2.65 billion the market is not yet pricing it as a certainty. This is the gene therapy pick with the widest swing in 2027.

The Avoids

8. Bristol Myers Squibb (BMY) | $62.86 | Market Cap $128.4B | MeziKd May 13

MeziKd (mezigdomide) is a real drug. SUCCESSOR-2 showed a 52 percent reduction in the risk of progression or death in relapsed myeloma, and the May 13 PDUFA is real. But it is the second CELMoD from a $128 billion company, immaterial to the stock no matter how it resolves. The MeziKd analysis has the data for the completists. For a catalyst trade, this is the same GSK-neladalkib trap from last year: the drug is good, the stock is uninvestable for catalyst returns.

9. Organogenesis (ORGO) | $1.41 | Market Cap $181M | ReNu April 24

ReNu, a cryopreserved amniotic allograft for knee osteoarthritis, has three randomized trials in over 1,300 patients and a Regenerative Medicine Advanced Therapy designation. The April 24 PDUFA is genuine. But this is exactly the one-and-done binary this list is designed to screen out. A $181 million micro-cap with thin liquidity, reimbursement risk, and no second shot if the BLA stumbles. Only the most aggressive accounts should touch it.

The Risks

Every name on this list carries binary risk, but the shape differs. IONS, SRPT, and TLX have commercial revenue that caps the downside. VKTX, RGNX, and QURE are pre-revenue and can lose half on a bad readout. ALNY is the only one where the risk is multiple compression, not a data miss. The gene therapy names share one structural risk: the FDA’s posture on single-arm and accelerated approvals has been tightening, and a single advisory committee can rewrite the odds for the whole class.

The Verdict

Ranked by risk-reward, the order is: TLX first for the revenue base with therapeutic upside, VKTX second for the obesity optionality, IONS third for the platform validation with a revenue floor. Then QURE and RGNX as the high-swing gene therapy binaries, sized at 1 to 2 percent each. SRPT is the contrarian hold for patient investors, and ALNY is the quality anchor for the core of a biotech portfolio.

Avoid BMY for catalyst trading and ORGO for everything except a lottery-ticket allocation. 2027 rewards the companies that already proved the platform works and still have another shot coming. Those are the seven names above.
