Novo's CagriSema Beats Tirzepatide 5 mg on Weight Loss, With a Dose Caveat

Novo Nordisk (NVO) said Sept 21 that CagriSema, its next-generation obesity drug, beat Eli Lilly’s tirzepatide on weight loss in a head-to-head Phase 3 trial, but the win came with a dose caveat investors caught immediately.

In REIMAGINE 5, which enrolled adults with type 2 diabetes, CagriSema 1.0/1.0 mg delivered 12.4% weight loss versus 9.1% for tirzepatide 5 mg, a statistically superior result, with non-inferior HbA1c reduction of 1.71% versus 1.67%. In a separate trial, REDEFINE 9, CagriSema produced 21% weight loss versus 2% for placebo in people with overweight or obesity.

The asterisk: tirzepatide was dosed at 5 mg, its lowest marketed dose, not the 10 mg or 15 mg doses that deliver its full 20%-plus weight loss. Beating the weakest dose of a rival is a real result, but it does not answer whether CagriSema can match tirzepatide at full strength.

CagriSema pairs semaglutide with cagrilintide, a long-acting amylin analog, in one weekly injection. The dual-target approach is Novo’s answer to tirzepatide, which combines GLP-1 and GIP agonism. Novo framed the results as up to 14.2% weight loss, with 43% of patients losing at least 15% and 24% losing at least 20%.

The amylin component is the differentiator. Cagrilintide suppresses appetite through a pathway distinct from GLP-1, which is why Novo argues the combination can deliver weight loss beyond what either hormone does alone. That dual mechanism is also why a widening set of challengers, from Viking’s VK2735 to oral GLP-1s in development, is racing to claim the same ground.

The market read the comparison for what it was: Novo shares slid through the week after the capital markets day, a signal investors noticed the dose-stacking. The genuinely encouraging number is REDEFINE 9’s 21% versus placebo, consistent with the 20.4% Novo reported in the earlier REDEFINE 1 trial, which confirms CagriSema is a potent obesity drug on its own.

Obesity is the most valuable market in biotech, and Novo and Lilly sit at the top of a duopoly investors are watching closely. CagriSema is central to Novo’s effort to hold its share as tirzepatide and a wave of challengers, including Viking’s dual GLP-1/GIP candidate VK2735 and oral options, crowd in.

What to watch: filing timing, and whether Novo runs a head-to-head against tirzepatide’s higher doses. Novo has not said when it will file, so the next concrete catalyst is a submission announcement rather than another data readout. NVO shares closed at $38.62.

What It Means for the Trade

The honest read of REIMAGINE 5: a real statistical win that does not settle the commercial question. Beating tirzepatide 5 mg by 3.3 points of weight loss is superior on paper, but 5 mg is the titration floor, and payers and prescribers know the 10 mg and 15 mg doses deliver the 20%-plus numbers that built Lilly’s franchise. If Novo commercializes on this head-to-head without testing the higher doses, the claim is contestable in every formulary negotiation. The more durable fact in the release is REDEFINE 9: 21% weight loss versus placebo, consistent with REDEFINE 1’s 20.4%, which confirms CagriSema’s own potency independent of the comparison. The catalyst path is short: no filing date disclosed, so the next tradable event is a submission announcement, then label language — where the dose caveat either gets disclosed in the trial design or invites a competitor advertising fight. The market has already voted on the nuance, with the stock sliding through the week after the capital markets day.

Mechanism and Market Context

CagriSema pairs a GLP-1 receptor agonist (semaglutide) with a long-acting amylin analog (cagrilintide) in a single weekly injection. Amylin acts through its own receptor system to suppress post-meal glucagon, slow gastric emptying, and reduce appetite — a pathway synergistic with GLP-1 rather than overlapping, which is why the combination pushes weight loss beyond either hormone alone. That dual-hormone frame is Novo’s structural answer to tirzepatide’s dual-agonist GIP/GLP-1 design, and the REDEFINE 9 result puts the two franchises in the same efficacy neighborhood in type 2 diabetes patients. The context is a market splitting into mechanism wars — dual and triple agonists, orals, and challengers like Viking’s VK2735 — where second-mover labeling claims are won or lost on head-to-head design choices. HbA1c non-inferiority (1.71% versus 1.67%) keeps the diabetes label intact alongside the obesity case.

Risks and What Could Go Wrong

  1. The 5 mg comparator caps the claim: an advertising or formulary fight over “lowest dose” framing is a real risk until a high-dose head-to-head exists.
  2. Novo has not announced a filing date — submission delay is the primary near-term timeline risk.
  3. Gastrointestinal tolerability stacked across two appetite-suppressing mechanisms can erode real-world persistence, as it has across the injectable class.
  4. Tirzepatide at full dose and next-generation challengers may reset the efficacy bar before CagriSema’s launch.
