If you are reading the EPS line on a biotech earnings report, you are reading the wrong line. That is the core of the 5-numbers earnings framework: a biotech print only moves the stock when it carries a pipeline, guidance, or runway signal. Q3 2026 earnings season opens mid-October, and the eight names below are the ones where the quarter can actually move the price, because each has a catalyst resolving within 90 days of the report. Ignore the GAAP headline on all of them. Here is the one number to watch on each, and what it means for the trade.

Four of these are launch-ramp reads (Merck, Lilly, Gilead, Bristol Myers), two are binary countdowns (Vertex to a November PDUFA, Moderna to a filing), one is a pipeline repair (Novo), and one is a credibility check (AstraZeneca). The signal type determines whether the quarter is a trade or a thesis confirmation.

1. Merck (MRK) at $148.78, $367B: the Welireg launch number

Merck reports in late October with the Welireg plus Lenvima approval already in hand. The FDA cleared the combination on September 24 for advanced ccRCC after PD-1/PD-L1 therapy, on LITESPARK-011, where it cut the risk of progression or death by 26 percent versus cabozantinib (HR 0.74, p=0.001) with median PFS of 14.6 versus 10.6 months and an ORR of 53 versus 40 percent. The number to watch is whether management quantifies the launch: initial weekly prescriptions, reimbursement wins, and any 2027 contribution. The I-DXd BLA was withdrawn on September 25 (see our withdrawal coverage), so Merck will instead frame the small-cell lung cancer program around the Phase 3 IDeate-Lung02 trial. At $367B, neither catalyst moves the stock alone; the print matters only as confirmation that the oncology pipeline is filling the Keytruda patent-cliff hole. See the MRK Q2 breakdown for the Terns acquisition charge context.

2. Moderna (MRNA) at $198.88, $79.4B: the intismeran filing timeline

Moderna is the one name here where the quarter is binary-adjacent, because the stock jumped 91 percent on the August 19 readout and has kept climbing since. The INTerpath-001 trial of intismeran (mRNA-4157) plus Keytruda met recurrence-free survival and distant metastasis-free survival in resected stage IIB-IV melanoma, the first positive Phase 3 for an individualized neoantigen therapy. The trial randomized 1,137 patients 2:1, and Grade 3 immune-related adverse events came in slightly lower on the combination at 10.6 percent versus 12 percent on Keytruda alone, a clean safety read for a mechanism regulators have never seen before. The Q3 number to watch is the regulatory path: when Merck and Moderna plan to file, and how they frame the 5-year Phase 2b data (HR 0.51 for RFS, HR 0.411 for DMFS). Secondary watch: the first MFlusiva flu season after its August approval. See the intismeran approval-push analysis and the MFlusiva launch analysis. At $79.4B, the re-rate has happened; the quarter now has to defend it.

3. Vertex (VRTX) at $526.19, $133.4B: the povetacicept PDUFA month

Vertex’s Q3 print is a countdown to the November 30 povetacicept PDUFA, the IgA nephropathy filing that anchors the renal franchise. Povetacicept is the TACI-Fc BAFF/APRIL dual inhibitor Vertex picked up in the Alpine Immune Sciences acquisition, and it is the central filing in the company’s 2027 story. The number to watch is specificity: management must name the PDUFA month and confirm the launch-readiness plan, not wave at “late 2026.” Secondary: the suzetrigine pain launch ramp and the Casgevy age-2 expansion. A vague answer on povetacicept timing is a dodge. See the VRTX Q2 earnings analysis.

4. Eli Lilly (LLY) at $1,183.46, $1.05T: the Mounjaro CV and Onswik launch

Lilly reports in late October with three launches to quantify: the Mounjaro cardiovascular-indication expansion, the Onswik (efsitora) weekly insulin launch, and the Inluriyo/Verzenio ESR1 breast cancer approval. The number to watch is obesity and diabetes franchise growth. Lilly is a $1.05 trillion company priced on GLP-1 dominance, so a deceleration in Mounjaro or Zepbound scripts matters more than any single FDA win. See the LLY Q2 earnings for the retatrutide filing context.

5. Novo Nordisk (NVO) at $38.80, $171.4B: the cagrisema read-through

Novo reports in early November, and the number to watch is cagrisema REIMAGINE5, the head-to-head against tirzepatide. Cagrisema pairs semaglutide with the amylin analog cagrilintide, and REIMAGINE5 is the readout that decides whether Novo closes the efficacy gap with tirzepatide on weight loss. After the monlunabant impairment and the ziltivekimab cardiovascular failures, Novo needs cagrisema to show it can compete on efficacy, not just on safety. Secondary: the delayed MIM8 hemophilia PDUFA timeline. The stock at $38.80 prices real skepticism about the pipeline; the quarter either confirms it or starts the repair. See the NVO Q2 pipeline impairment breakdown.

6. Gilead (GILD) at $150.93, $187.1B: the Bixlenvo launch and base-business mix

Gilead’s Q3 number to watch is the Bixlenvo HIV launch against the base-business-versus-acquisition-charge mix that spiked Q2. The Trodelvy CHMP read-through for TNBC is the secondary. Gilead faces the same patent-cliff-to-growth transition as Merck, and the market wants to see the HIV franchise, Biktarvy plus the new launches, hold the line. See the GILD Q2 earnings.

7. Bristol Myers Squibb (BMY) at $62.86, $128.4B: the Zenbecus launch and MeziKd setup

BMY reports in late October with the Zenbecus (iberdomide) launch underway after its August 13 approval. The number to watch is the MeziKd pre-PDUFA commentary: management will be asked how two CELMoDs coexist in one myeloma franchise ahead of the May 13, 2027 decision. At roughly 9x forward earnings, BMY is priced for a declining Revlimid base and a pipeline that must deliver. See the BMY Q2 earnings.

8. AstraZeneca (AZN) at $166.58, $258.3B: the Etcmah launch after SERENA4

AZN’s Q3 number to watch is the Etcmah (camizestrant) ESR1 launch, which now has to carry the franchise after the SERENA4 first-line failure. The market wants to know whether the accelerated approval converts to real prescriptions and whether the first-line miss caps the drug’s ceiling. At $258B, it is a pipeline-credibility question more than a quarterly one.

What a dodge sounds like

On every call, management will be asked specific questions. Here is what the real answer sounds like versus the dodge. Vertex must name the povetacicept month, not say “the review is progressing.” Novo must give the MIM8 timeline in months, not “we remain in dialogue with regulators.” Moderna must commit to a filing window for intismeran, not “we look forward to engaging the agency.” If you hear the hedge, the catalyst is slipping. If you hear the number, the quarter is doing its job. The tell is almost always in the Q&A, not the prepared remarks, because management scripts the prepared remarks and cannot script the follow-up. For any pre-revenue name on this list, run the cash-runway math from the 10-Q and flag dilution risk under 12 months, per the dilution survival guide. Read the call transcript the same way, with the earnings-call guide.

The verdict

The quarter that matters is the one where a catalyst resolves within 90 days of the report. By that test, the two names to position around are Moderna and Vertex, where the Q3 print is a genuine countdown to a binary (the intismeran filing, the povetacicept PDUFA). Merck, Gilead, BMY, Lilly, and AZN are launch-execution reads on mega-caps where the number confirms a thesis but does not create one. Novo is the contrarian watch: a beaten-down stock that either confirms the skepticism or begins the repair. Size the Moderna and Vertex trades small, because a binary resolves within a quarter of the print and both stocks already carry the expectation; the mega-cap reads are holds, not trades. Open the 5-numbers earnings framework before you open any of these calls, and ignore the GAAP EPS on all of them.
