SABCS 2026: 5 Breast Cancer Abstracts That Move Stocks

Every biotech investor screens ASCO and ESMO. Almost no one screens the San Antonio Breast Cancer Symposium, which is exactly why it is where the edge lives. SABCS 2026 runs December 8 to 11, but the trade does not fire at the podium. Abstract titles drop October 30 and the full abstract text goes live November 23, roughly two weeks before the meeting. That November 23 release is the trade date, and the positions are entered before the presenter ever steps on stage. By the time a session ends, the stock has already gapped.

This is the one major Q4 oncology meeting the site has not previewed. Prior previews have covered ESMO, ASH, SITC, and ACR. SABCS is where the breast cancer data that does not fit ASCO or ESMO lands: the oral SERD war, the PI3K/AKT next wave, and the TROP2 ADC read-through. Here are five names ranked by stock-moving potential, not by market cap.

1. Olema (OLMA, $8.75, $766M cap): the oral SERD pure play

Olema is the only investable small cap in the oral SERD war, and it is priced like the market has already forgotten it. Palazestrant, its complete estrogen receptor antagonist, degrades the receptor outright rather than merely blocking it, which matters because ESR1 mutations make the receptor resistant to aromatase inhibitors in roughly half of these patients. It has two Phase 3 trials. OPERA-01 tests it as a monotherapy in second and third line ER+/HER2- metastatic disease; enrollment completed in August and top-line data now land in the first quarter of 2027. OPERA-02 tests it with ribociclib in the first line. The SABCS screen is the updated palazestrant plus ribociclib Phase 1b/2 dataset, the same study that posted a 73% six-month progression-free survival rate in all comers and 81% in ESR1-mutated patients at the 2024 meeting.

The re-rate thesis is the spread between the science and the price. Palazestrant’s combo data so far matches or beats the approved oral SERDs, yet Olema trades at $766 million against Lilly’s Inluriyo, the class benchmark, which set the ceiling the rest of the field is measured against. A clean combo read alongside the Q1 2027 OPERA-01 binary sets up a re-rate toward $1.5 billion to $2 billion. The risk is the mirror image: $461 million of cash against a Q1 readout means a monotherapy miss cuts the stock in half. This is a 1% position sized for a binary, not a core holding.

2. Celcuity (CELC, $79.52, $3.89B cap): the PIK3CA-mutant expansion

Celcuity already has one approval. REVTORPYK (gedatolisib) won its PIK3CA wild-type label July 14, and the company filed an sNDA August 26 to extend it into PIK3CA-mutant disease, which is roughly 40% of the HR+/HER2- subtype. The filing rests on the VIKTORIA-1 mutant cohort, where the gedatolisib triplet cut the risk of progression or death by 50% versus Novartis’ alpelisib (hazard ratio 0.50, p less than 0.0001) with a median PFS of 11.1 months versus 5.6 months. That is the cleanest head-to-head win in recent breast cancer history.

The SABCS screen is the updated VIKTORIA-1 mutant-cohort data, which Celcuity has said it will present at fourth quarter medical conferences. What moves the stock is not another PFS number; it is the label language and the durability signal that shape the sNDA decision. Approval roughly doubles the addressable market, and the full REVTORPYK thesis already priced a launch. The mechanistic edge is the breadth: gedatolisib hits all four class I PI3K isoforms plus mTORC1 and mTORC2, where alpelisib only blocks the alpha isoform, which is why the head-to-head win is credible rather than a trial-design artifact. The risk is the 72% stomatitis rate on the triplet and a launch with no revenue yet. At $3.89 billion, a priority review date plus a clean SABCS read re-rates this toward $5 billion; a weak safety discussion on the podium caps it.

3. AstraZeneca (AZN, $166.58, $258B cap): the SERD that won and then missed

AstraZeneca is the class’s most confusing story, and SABCS is where the confusion resolves. Camizestrant, sold as Etcamah, won accelerated approval September 4 on SERENA-6, the ctDNA-guided switch trial that cut the risk of progression or death by 55% (hazard ratio 0.45) with a median PFS of 16.8 months versus 9.2. Then SERENA-4’s first-line failure capped the drug to the ESR1-mutant niche. The SABCS screen is the full SERENA-4 dataset and any confirmatory SERENA-6 analysis.

This is a mega-cap, so nothing here moves AZN. The value is the read-through: if the full SERENA-4 data shows a survival or quality-of-life signal, the first-line ceiling on the whole oral SERD class reopens. If it does not, the class stays boxed into ESR1-mutant disease and Roche’s giredestrant PDUFAs become the benchmark. Watch AZN for the class signal, not the ticker.

4. Eli Lilly (LLY, $1,183.46, $1.06T cap): the SERD with the fuller label

Lilly’s imlunestrant, sold as Inluriyo, is the oral SERD the market actually rewards. It won monotherapy approval September 2025 and a full approval in combination with Verzenio in September 2026, doubling median PFS to 11.1 months versus 5.5 in the ESR1-mutant setting (hazard ratio 0.53). The SABCS screen is EMBER-3 follow-up and any EMBER-4 adjuvant read-through, which reads out in 2027.

At a $1.06 trillion market cap, Inluriyo is immaterial to Lilly’s stock. The value here is the class benchmark: Inluriyo plus Verzenio is the number every other oral SERD has to beat, and its adjuvant push is the reason the first-line question matters so much. Ranked fourth because the data shapes the class, not the ticker.

5. Gilead (GILD, $150.93, $187B cap): the TROP2 ADC read-through

The TNBC/TROP2 slot goes to Gilead because Trodelvy is the only ADC with four positive Phase 3 trials in HER2-negative breast cancer. Trodelvy plus Keytruda won first-line PD-L1-positive TNBC approval in June on ASCENT-04, cutting the risk of progression or death by 35% (hazard ratio 0.65) with median PFS of 11.2 months versus 7.8. The SABCS screen is the longer-term ASCENT-04 and ASCENT-03 analyses, including PFS2 and biomarker subgroups.

This is the ADC field’s broadest read-through, and it is a mega-cap, so the trade is not GILD. It is what the data says about TROP2 sequencing and where the next B7-H3 and next-generation ADC programs slot in. Screen it for the class signal and move on.

The mega-cap filter

Enhertu follow-ups will dominate SABCS headlines and move zero basis points, because AstraZeneca and Daiichi Sankyo are too big for a HER2-low subgroup analysis to matter to the share price. Roche’s giredestrant is the cleaner example of the same problem: the November 30 adjuvant PDUFA will already be decided before SABCS begins, and the December 18 ESR1-mutant PDUFA lands a week after it ends. Neither decision is a SABCS trade. The conference will not be where giredestrant moves; the FDA will be, and both dates sit outside the meeting window. Exclude all of it.

The playbook

Abstract release, November 23, is the trade date, not the podium. The full playbook is in the conference trading guide: enter on the abstract, size each binary at 1% to 2% of the portfolio, expect implied volatility crush after the release, and apply the 24-hour rule. Do not hold through the meeting for a reaction that already happened.

The verdict

Two of these five names actually re-rate. Olema is the reward-per-dollar play: a $766 million oral SERD pure play with a Q1 2027 monotherapy binary and combo data at SABCS that could justify a double. Celcuity is the safer version of the same idea: an approved drug with a pending mutant-label expansion and SABCS data that shapes the decision. Both are 1% positions, not convictions. The three mega-caps are the class context, not the trade. Screen them for what their data means for Olema, Celcuity, and the oral SERD and PI3K field, then buy the two names where a single abstract still moves the stock.
